The Appointment and Acknowledgement form, Repertoire Reporting form, Authorization form and Certificate with Respect to Residency make up the Assignment Package. The Assignment Package, when completed, authorizes RACS to collect on your behalf.
The documents in this section are stored in Adobe pdf format. To view documents offline (to save these documents to your desktop) right click on the icon and select "Save target as" (Mac users hit option click).
